Inference-based Cognitive Behavioral Therapy (I-CBT)


Inference-Based Cognitive Behavioral Therapy (I-CBT) is a specialized form of cognitive therapy primarily developed to treat Obsessive-Compulsive Disorder (OCD). Unlike traditional CBT, which often focuses on challenging the content of thoughts and reducing compulsions through exposure, I-CBT focuses on how a person arrives at obsessive doubts in the first place.


The Core Idea

I-CBT proposes that OCD begins with a false inference (a mistaken conclusion) rather than a realistic concern.

For example:

  • A person touches a clean doorknob.
  • There is no actual evidence of contamination.
  • OCD creates a doubt: "What if there are dangerous germs on my hands?"
  • The person starts treating this imagined possibility as if it were reality.
  • Anxiety increases, leading to compulsions such as excessive handwashing.



I-CBT helps individuals recognize when they have left reality-based reasoning and entered what therapists call the "inferential confusion" process—giving more weight to imagined possibilities than to direct evidence.
