Inference-based Cognitive Behavioral Therapy (I-CBT)

Inference-Based Cognitive Behavioral Therapy (I-CBT) is a specialized form of cognitive therapy primarily developed to treat Obsessive-Compulsive Disorder (OCD). Unlike traditional CBT, which often focuses on challenging the content of thoughts and reducing compulsions through exposure, I-CBT focuses on how a person arrives at obsessive doubts in the first place.

The Core Idea

I-CBT proposes that OCD begins with a false inference (a mistaken conclusion) rather than a realistic concern.

For example:

  • A person touches a clean doorknob.
  • There is no actual evidence of contamination.
  • OCD creates a doubt: "What if there are dangerous germs on my hands?"
  • The person starts treating this imagined possibility as if it were reality.
  • Anxiety increases, leading to compulsions such as excessive handwashing.

I-CBT helps individuals recognize when they have left reality-based reasoning and entered what therapists call the "inferential confusion" process—giving more weight to imagined possibilities than to direct evidence.

Treatment Approaches

My aim is to offer services to individual adults through specialized treatment methods, enabling the client and me to select a therapeutic approach uniquely suited to their needs.

  • Inference-based Cognitive Behavioral Therapy (I-CBT)

This evidence-based approach views OCD as a reasoning disorder characterized by obsessional doubts and compulsions. I-CBT provides symptom relief by guiding clients to focus on actual sensory evidence and present-moment experience. Through this process, clients develop the ability to trust themselves and rely on what they know to be true, rather than imagined possibilities. This treatment promotes recovery without the need for prescribed exposures.

  • Exposure and Response Prevention (ERP)

Exposure and Response Prevention (ERP) is a type of Cognitive Behavioral Therapy (CBT) that helps individuals confront distressing obsessional thoughts, urges, or images. During the exposure process, individuals refrain from performing compulsive behaviors aimed at reducing their discomfort. This approach helps reduce distress over time and teaches individuals that their fear or anxiety is unwarranted.

  • Acceptance and Commitment Therapy (ACT)

Acceptance and Commitment Therapy (ACT) is an evidence-based form of Cognitive Behavioral Therapy (CBT) that emphasizes aligning actions with personal values. It incorporates acceptance and mindfulness techniques to enhance psychological flexibility and improve overall well-being.
